Passport Services Moved to Library

Passport services are not available at Town Hall; passport applications are accepted at the Pembroke Library effective December 3, 2018.

 

Passports will be by appointment with some night and Saturday hours and passport documentation will be the same.  

 

The fee paid to the State Dept varies with the type of service requested; please see this listing of passport fees.  Please note that the Town fee is a separate check of $35.00 per application; as set by the State Dept. (Passport Agency.)

Download an application and complete questions 1 through 21 in black ink and DO NOT SIGN APPLICATION until you meet with an acceptance agent as it must be signed in their presence.

New passports require original, long-form birth certificates or original naturalization papers.

Photographs are not available, residents must bring (2) two - 2" by 2" passport photographs when they submit the application.

All applicants using the DS-11passport application must be seen by the passport acceptance agent, including children under age 16 years, who should be accompanied by *two parents. This information is stated on the front of the DS-11 under Special Requirements for Children and reads: “As Directed By Regulation 22 C.F.R. 51.21 and 51.28: Each minor child applying for a U.S. passport book and/or passport card must appear in person.”  Children age 16 and 17 years should be accompanied by one parent.

*If only one parent is available to accompany a child under age 16 years, please refer to the front of the DS-11 where it reads Special Requirements for Children and read the requirements for the use of a DS-3053 form to go along with the DS-11 passport application.
