The Town of Pembroke's open enrollment for FY 2022 will begin on April 26, 2021 and end on May 14, 2021. Enrollment forms for most plans along with information for each plan is listed next to or below the coverage option. Life insurance enrollment or changes to your existing policy are not available during this enrollment period. Boston Mutual Life Insurance and Mayflower Municipal Health Group (MMHG) hold a separate enrollment at their discretion. If you are enrolling as a new employee or due to a qualifying event please contact the Treasurer's Office at treasurer@townofpembrokemass.org or 781-293-3893 for further instruction.  

Availability of Summary Health Insurance

As an employee, the health benefits available to you represent a significant component of your compensation package. They also provide important protection for you and your family in the case of illness or injury. Your plan offers a series of health coverage options. Choosing a health coverage option is an important decision. To help you make an informed choice, your plan makes available a Summary of Benefits and Coverage (SBC), which summarizes important information about any health coverage option in a standard format, to help you compare options. The SBC's are available on this page below or you can request one free of charge by calling the Treasurer's Office at 781-293-3893.

FY 2022 MMHG PPO & HMO Plan Comparison (for specific plan questions please click here for insurance company contact information)***Family plan coverage requires additional documentation. Please return to the Treasurer's Office the following documents along with your enrollment form and the last page of the MMHG Employee Manual: certified marriage certificate to cover spouse or divorce decree to cover a former spouse (please refer to MMHG Employee Manual for specific pages required from the decree) and copies of birth certificates for dependent children up to age 26.***

Learn to Live (L2L) (Newly offered program that provides free confidential online therapy program for all employees and their family members age 13+ which uses cognitive behaviorial therapy techniques (CBT) to cope with stress, anxiety, substance use, social discomfort and trouble sleeping)
