Employee Benefit Information

The Town of Pembroke's open enrollment will begin May 1, 2020 and end on May 15, 2020. Due to the current pandemic, we advise employees to contact our office at treasurer@townofpembrokemass.org or 781-293-3893 for further instruction if you wish to enroll, change or cancel any coverage. Please expect a response to your request within 24-48 hours. We kindly request forms listed below to be returned to our office for processing unless otherwise instructed. Please note if enrolling in family coverage our health group requires a marriage certificate or divorce decree if covering a former spouse and birth certificates for covered dependents. Life Insurance is not offered during this enrollment period. Thank you for your patience as we navigate through this challenging time.
