Open enrollment will be held April 15, 2024 through May 15, 2024 with plans being effective July 1, 2024. Life insurance will not be available during this open enrollment.                                                                               Welcome to the benefit information page for new and current employees working for the Town of Pembroke. If you are enrolling in benefits, as a new employee or due to a qualifying event, please contact the Treasurer's Office at 781-293-3893 or email Jennifer Grasso at jgrasso@townofpembrokemass.org for further instruction. Enrollment forms, rate information and plan material for coverages offered can be found below. Please note, enrollments and changes will not be accepted for existing employees unless it is during our annual open enrollment, due to a qualifying event or to enroll with a 457(b) or 403(b) plan provider. Please view the 457(b) and 403(b) section below for further instruction. 

Availability of Summary Health Insurance

As an employee, the health benefits available to you represent a significant component of your compensation package. They also provide important protection for you and your family in the case of illness or injury. Your plan offers a series of health coverage options. Choosing a health coverage option is an important decision. To help you make an informed choice, your plan makes available a Summary of Benefits and Coverage (SBC), which summarizes important information about any health coverage option in a standard format, to help you compare options. The SBC's are available on this page below or you can request one free of charge by calling the Treasurer's Office at 781-293-3893.

Health insurance carriers are available to answer specific plan questions during open enrollment. When contacting our carriers, please identify yourself as a Town of Pembroke employee, under Mayflower Municipal Health Group, and have the plan name ready for your inquiry. FY 2025 MMHG PPO & HMO Plan Comparison ***Family plan coverage requires additional documentation. Please return to the Treasurer's Office the following documents along with your enrollment form and the last page of the MMHG Employee Manual: certified marriage certificate to cover spouse or divorce decree to cover a former spouse (please refer to MMHG Employee Manual for specific pages required from the decree) and copies of birth certificates for dependent children up to age 26.***
